Exalon Care Homes Limited will consider emergency admissions in extreme circumstances.

Our strict criteria require a current Community Care assessment, Care Plan and any other relevant documentation to be made available to us before the admission decision is taken. We expect them to be provided by the Care Manager/Social worker, potential service user or previous placement as applicable. If we can meet the specific needs of the potential service user, written confirmation will be sent to the service user and/or the appropriate representatives offering a placement under a 3 month trial period.
