The service has a detailed and comprehensive assessment process for new service users. We encourage a potential service user to visit the home on different days and times, thus participating in meals, social interaction and activities where appropriate and gaining a good idea of how they would be supported within the home. Families are encouraged in supporting and assisting with this transitional period.

Full account is taken of the Community Care Assessment and Care Plan from the care manager or previous placement in preparing an initial Care Plan prior to admission. The first week of a new placement will also be used for a more detailed assessment of a service user’s needs. Careful records will be kept during that period and the Plan will be adjusted if necessary in consultation with all parties.

In addition, a block placement respite facility may be provided within the homes, which offer care packages for periods not less than 5 days. Potential users of this service would be offered the same care and support through planning, as those in permanent placements.
